News and Announcements

Reminders, as we approach the end of the year:

  • If you have worked for the University during 2015 and have moved, don't forget to report your change of address to the Office of Payroll and Employee Benefits by December 31, 2015 to ensure your tax forms are delivered to your current address.
    • You may change your address by using one of the following options:
      • Go to Self Service Banner (SSB), Personal Information tab and update your address.
      • Fill out the Change of Address form and return it to the Payroll and Employee Benefits Office via 
        • Email at
        • Fax to 765-285-6612
        • Dropping it by our office in the Administration Building, Room 29
      • Call us at 765-285-8461 
  • Your 2015 Form W-2 will be mailed no later than January 31, 2016.
  • You will also receive a new form this year called Form 1095-C Employer Provided Health Insurance Offer and Coverage due to the Health Care Reform Act.  This form will also be mailed no later than January 31, 2016.

We do this by: Providing leadership and guidance in the implementation, interpretation and equitable administration of University policies and procedures, and Creating an environment of mutual respect through partnering with vendors, government entities, campus departments and other members of the Ball State community.
