Certificate of Insurance

A Certificate of Insurance (COI) is a document used to verify an entity is insured.  Outside parties often request that the University confirm various types and levels of insurance by issuing a COI.   The Office of Risk Management will issue a COI for Ball State departments as required by written contract or agreement provided proper contract procedures are followed.

To request a COI, simply email the Office of Risk Management.  Include the name and address of the certificate holder (the entity requesting the COI) and a copy of the insurance clause that requires the COI.    Since the COI is issued by the University’s insurance carrier, please allow 3 -5 business days for processing.
