Health Form
Indiana State Law requires students to turn in their immunization information prior to classes starting, but allows a one academic term grace period. The law prohibits the university from allowing students to register for a second term if they have not turned in their
health form. A hold is placed on the accounts of all students who do not turn in their form. This hold prevents students from registering from classes for a subsequent semester until they turn in all their information.
