The Ball State University Study Abroad program requires that students come in for a consultation and medical clearance. The purpose of this requirement is to identify any medical needs you are likely to have during your time traveling, and to make sure you are educated about precautions you can take for your health while you are away, such as recommended or required immunizations. At this time, each student needs to complete the pre-travel assessment form and submit this to the Health Center prior to being seen. Once this is received by the Health Center, then you will be able to make an appointment. Please call the front desk at 765-285-8431 to make an appointment.
We are also asking that students who are traveling abroad for non-university sponsored programs such as vacation or mission trips make an appointment for a consultation as well. You will need to fill out the pre-travel assessment form also-use this link above to access it.
