Obtaining Medical Records

The student health center at Ball State University is administered by IU Health Ball Memorial Hospital.  We follow the policies and procedures established by that health system for the protection of medical information.  Those policies and procedures, in turn, are established to meet the requirements of the Federal Health Information Portability and Accountability Act.  In order to request your records, you must submit a written request for your records.

In order to obtain medical records, follow these directions:

• Contact the Health Center at 765-285-8431 or healthcenter@bsu.edu and request a copy of a "Authorization to Release Health Information" form, or come by in person  to make your request.
• There is a fee schedule for copying records at the top of the form.  At this time, the cost of providing these records is covered by the operational budget of the Health Center and there is no charge to students for providing records to them.
• There are complete instructions on completing the form printed on the back of the form except that it does not include the address for returning the form to us.

We are typically able to fulfill requests within 10 business days. If you need your records more urgently than this, please call us by phone and alert the staff to the urgent nature of your request, and we will do our best to expedite your request.
