Field Trips
Faculty planning field trips for their class(es) or organization(s) need to complete the following forms:
- Request for Educationally Related Activity
- (This form needs to be completed at least 10 days prior to the start of the trip and filed with the designated administrator.)
- Field Trip Roster
- (This form must have administrative approval prior to the trip. A copy should be given to the main office and to the bus driver if such transportation is used.) Field trip rosters should be given to teachers as necessary.
- Individual Permission Slip
- (Each student must have a permission slip signed by their parent or guardian and kept on file prior to a field trip which leaves the BSU campus.)
- Parental Authorization Form for Overnight or Out of Country Travel
- (This form must be completed by the parents/guardians and must be notarized.)
- Requests for BSU vehicles are to be made through the financial coordinator. A Travel Authorization Information form must be completed if the faculty member is driving a BSU vehicle for traveling on a bus.
Students must be in communication with teachers of classes they are missing that day for homework assigned while on the field trip.
In addition, note tasks to be completed at the bottom of the Request for Educationally Related Activity Form. Be sure to notify the cafeteria and other personnel who are typically scheduled to work with students.
