W-9

A W-9 is an IRS form used to request taxpayer name, address and tax identification number. It is often necessary this form be completed before the university can pay an individual or company for goods or services. If payment is for performances or services, a W-9 is necessary before payment will be made. If presenting a contract to a potential performer or service-provider, please include a W-9 with your contract. Submit the completed W-9 with your request for payment.
