1098-T Form Information

Please note, effective for the 2012 tax reporting year, Ball State is providing allowable charges in Box 2, and Scholarships and Grants in Box 5.  In previous years, we provided allowable payments in Box 1.  Even though our reporting has changed, it  does not change your requirement to report to the IRS allowable payments made in 2012.
