Form 1098-T

Form 1098-T is typically mailed to the mailing address (or local address if home address was not provided) for every student who took credit classes during the calendar year, unless your student signed up to receive their 1098-T form electronically. The IRS requires Form 1098-T be postmarked by January 31 of the next calendar year.  If you would like to receive your 1098-T forms electronically, please click here for additional information.

To view prior 1098-T forms, please go to the ECSI website to retrieve a copy.  When logging into the ECSI website, you will enter the following information:

  • School Code (Ball State's school code is Z8)
  • Account Number (student's social security number)
  • Pin/Password

If you do not know your pin or password, click on the Need help logging in? link.  You will be prompted to provide your First and Last Name, the last four digits of your social security number, your date of birth, and your zip code.  Once you select the submit button, the system will provide to you your login information.

From the main account page, you may:

  • Select the Tax Information link to select a 1098-T form to view and print.
  • Select the Information Release Form link to indicate individuals you wish to be able to receive 1098-T information.

Authorized individuals indicated on the Information Release Form are able to contact ECSI representatives by phone and through a live chat server in order to receive answers to questions regarding their student's 1098-T form.  If a student provides his ECSI login information to his authorized users, they can log into the ECSI website and retrieve current and prior 1098-T forms. 

If you are not enrolled in the current spring semester, it is after February 15, and you did not receive your Form 1098-T for the previous tax year, please go to the ECSI website to retrieve a copy.

Legal Guardians

As the legal guardian of a former student, you may request a duplicate Form 1098-T be sent to you provided your guardianship can be verified. If guardianship and permissions in the new proxy system cannot be verified, the student must make the request. Please visit proxy access for information on providing this permission. If you are set up as a proxy with a Passphrase for your student's account, please e-mail the following information to bursar@bsu.edu:

  • your full name
  • relationship to student
  • student's full name
  • student's ID number
  • Passphrase for the proxy system
  • address to which the statement needs to be mailed
  • daytime phone number at which you can be contacted if there are questions
