Forms 1098-T are created and mailed to students by Ball State University’s partner, ECSI. As required by the Internal Revenue Service, Forms 1098-T for tax year 2015 will be postmarked by February 1, 2016.
The form is typically mailed to the mailing address (or local address if home address was not provided) for every student who took classes during the calendar year, unless your student signed up to receive their Form 1098-T electronically. The amount reflected in Box #2 on the Form 1098-T is only the qualified tuition and related expenses charged to your student account during the 2015 tax year. This is not necessarily the amount which was paid in the tax year. The parent, guardian, or student is responsible for determining and reporting to the IRS the amounts that were paid out-of-pocket or with borrowed funds during the 2015 tax year toward qualified tuition and related expenses on their personal income tax filing.
The amount reported in Box #5 represents the scholarships and grants received and processed to the student account for the calendar year. Amounts of scholarships reported on your Form 1098-T are for determining eligibility for the American opportunity tax credit and lifetime learning credit only, and is not for determining the taxability of such scholarships. Your scholarships may or may not be taxable. For more information, please refer to IRS Publication 970.

Authorized individuals indicated on the Information Release Form are able to contact ECSI representatives by phone and through a live chat server in order to receive answers to questions regarding their student's Form 1098-T. If a student provides his ECSI login information to his authorized users, they can log into the ECSI website and retrieve current and prior Forms 1098-T.
If you are not enrolled in the current spring semester, it is after February 15, and you did not receive your Form 1098-T for the previous tax year, please go to the ECSI website to retrieve a copy.
