Note to Parents: If your child has already had some shots, a record of all previous immunization shots MUST accompany the child. If any adult other than the parent or guardian takes the child for shots, he or she must have WRITTEN PERMISSION from the parent or guardian. The note should include the names of the parent, child, adult accompanying the child and clinic date.

Kindergarten booster shots are given at age 4 to 6 years. The 2nd MMR and HEP-B series are given to children through 18 years of age. This requires a complete shot record with proper adult permission.
