Voluntary Benefits

Voluntary Benefits are 100% employee-paid and payroll deductions for premiums are post-tax.  Retirees are not eligible to enroll in voluntary benefits.

Vision Service Plan (VSP)

You may only enroll in or cancel your VSP benefits during the Open Enrollment period.

The Hartford

Voluntary Life and Short-Term Disability (STD) insurance is only available to new hires (must enroll within the first 31 days), and during Open Enrollment for current hires who are benefit-eligible.  You may cancel these benefits at any time.  

Pre-populated enrollment forms will be mailed to your home from The Hartford during Open Enrollment.  If you do not receive one, please contact our office at 765-285-8461.

Voluntary (Supplemental) Life Insurance (pdf) - Benefit highlight sheet for voluntary life insurance.  Guarantee Issue Amounts are being made available during this Open Enrollment to all benefit-eligible employees.  You may enroll up to the Guarantee Issue Amount without a medical exam.  Here are the new amounts related to voluntary life insurance:

  • Maximum amount of voluntary life insurance for the employee has increased from $300,000 to $500,000; the Guarantee Issue Amount for the employee has increased from $150K to $200K
  • Maximum amount of voluntary life insurance for spouses and Same-Sex Domestic Partners (SSDP) has increased from $150K to $250K; the Guarantee Issue Amount for spouses and SSDPs has increased from $25K to $50K
  • Dependent children are eligible for either $5K or $10K in coverage

Voluntary Short-Term Disability (STD) Insurance (pdf) - STD is now being offered to Faculty, Professional and Staff employees.  Service employees are not eligible because they already receive STD benefits that are University-subsidized. 


Enrollment/Cancellation forms for Allstate benefits are not available online.  Forms are available in the PEB office and will also be available at all of the Open Enrollment events.

*Please note that the MetLife Auto and Home products are offered year-round.

All of the voluntary benefit vendors will be attending the Benefits Fair scheduled during Open Enrollment on Tuesday, November 11th in the Student Center Ballroom from 9:00 am to 5:00 pm.
