Certificate of Insurance

A Certificate of Insurance (COI) is a document used to verify an entity is insured. Outside parties often request the University to confirm various types and levels of insurance in connection with business agreements. The Certificate Holder is the individual organization requesting the COI.

Use ORM 7 Certificate of Insurance Request to obtain the COI from the Office of Risk Management.  Since the COI is issued by the University’s insurance carrier, please request it two weeks in advance of needing it.
