Chronic Disease Self-Management
The Chronic Disease Self-Management Program, developed by Stanford University, is appropriate for individuals who have any chronic disease including diabetes, asthma, arthritis and hypertension. The six session workshop addresses:
- techniques to deal with problems such as frustration, fatigue, pain and isolation
- appropriate exercise for maintaining and improving strength, flexibility, and endurance
- appropriate use of medications
- communicating effectively with family, friends, and health professionals
- nutrition
- how to evaluate new treatments
Employees or spouses attending 5 of 6 sessions complete the "Take Charge" step toward the wellness incentive.
