New Wellness Incentive

In 2012 - 2013, benefit eligible employees and covered spouses or same sex domestic partners have an incentive option. Upon completion of your wellness program, you can select either a $90 Ball State Working Well voucher or a $75 MasterCard gift card.

Complete the three steps listed below between July 1, 2012 and June 30, 2013 to attain your incentive:  

  1. Know Your Numbers call 765-285-9355 to schedule a biometric screening. You will need the screening values to complete step 2, you may also use recently attained lab values.
  2. Complete the Life Health Assessment:   Online Assessment  (eligible spouse or same sex domestic partners call 765-285-9355 and a paper version will be mailed to your home) 
  3. Take Charge by engaging in a positive lifestyle! Find organized or self-directed programs that meet your needs below.

To complete step 3 Take Charge, you can complete one of the programs listed below:

  • 6 - week walking program
  • Diabetes education
  • Chronic Disease Self Management program
  • Health Coaching program
    • iCan Quit
    • iCan Relax
    • iCan Thrive Cardiovascular Health Program
    • iCan Change Weight Management Program

    Or a 6 week total combination of the following:

     Tracking is easy.  For most programs, Working Well will track your participation. Paper tracking logs are available for self-directed programs.

    What will my $90 Ball State Working Well voucher buy?

    You can use your voucher toward the purchase of one of the following Ball State services:

    Note: Participants are responsible for any expense greater than the face value of the voucher.
