Undergraduate Grade Records

• An overall undergraduate GPA of 2.75 is required for admission to the Ball State University Graduate School; an overall undergraduate GPA of 3.00 is preferred.
• A DPD GPA of a 3.0 is preferred.
• If any graduate course work has been completed, please provide a transcript.

• Please list all earned during college career.

Sports and Extracurricular Activities

• Please list all activities throughout college career including athletic teams, student organizations, and professional memberships.

Certifications

• Please list all certifications currently valid.

Work Experience

• Please note that references from college instructors and work supervisors will be considered more strongly than references from others.
