Health Requirements

Health requirements for students at Ball State University exist primarily to protect the student body as a whole from infectious disease. Requirements are different for international students and domestic students (those from the United States).

International graduate and undergraduate students must meet all the same health requirements as undergraduate domestic students and have an evaluation for tuberculosis (TB) performed in the United States. Either a TB skin test or a Quantiferon Gold blood test performed in the United States is acceptable. Testing will be offered as part of international student orientation prior to beginning classes. Each student will be charged for the TB test, and it will be billed to your bursar's account. You will also need to have a pre-enrollment health examination. Download the Health History Form and have your physician complete the Report of Immunizations.  You will then complete the remaining sections of the form and send it to the health center.

We recommend that you complete the following health form with your doctor and mail it to the address below:

Health Insurance Requirement

Ball State University requires that all international students carry health insurance.  Unlike most countries, the United States does not have a national health insurance plan that covers all individuals.
