Health Insurance

Ball State University requires that all international students carry health insurance. Unlike most countries, the United States does not have a national health insurance plan that covers all individuals. Emergency room treatment is available to all (but will be extremely costly without insurance) but treatment from a doctor for other non-emergency services is in most cases not available without health insurance.

Ball State endorses a HTH Worldwide health insurance plan for international students.
The HTH Worldwide health insurance plan will provide you with service and protection in the event you become ill or injured during your studies. HTH Worldwide also provides you and your family with valuable information to help you use and understand your health insurance program.

Please note all international students are automatically enrolled in the HTH Worldwide plan unless they have an exemption or apply for a waiver.

Waiver: If you currently have health insurance that meets or exceeds the Ball State plan, you can apply for a waiver through the insurance company.

Exemption: Students who receive a full scholarship from a Government or Agency Sponsor that provides health insurance coverage that meets or exceeds the Ball State plan are exempt from coverage. Students must provide proof of funding and insurance from an enrollment financial guarantee or similar legally binding sponsorship letter. 

Enrollment:

  • Ball State will automatically enroll you in the Health Insurance program if you are issued a Ball State I-20 or DS-2019. You will be enrolled for the entire time you have an active SEVIS record. 
  • The Health Insurance plan begins on August 15th and lasts until August 14th the following year.
  • Dependents (Children and Spouses) can be signed up by mail using the following forms: Dependent Medical Insurance Enrollment Form 2013-2014 Although dependents are not required to purchase the health insurance, we STRONGLY SUGGEST that you provide health insurance for your family.
  • After you are enrolled, you will receive an email from HTH with instructions on how to register, to access your insurance ID Card and other important information about their online tools and services for access to this important healthcare.

* Use the Dependents form to enroll family members. Dependents cannot be billed through the Bursar account. 

  • You will be billed for the insurance through your Bursar Account. 
    • For returning students, you will see a bill on your account beginning in Early August. If you do not intend to return to Ball State AND your program has not ended on your I-20 or DS-2019, you must notify an International Student Advisor in order to ensure that you are not billed. 
    • For new students, you will see a bill during your first two weeks of classes.
    • Beginning Fall 2013, you will be billed once in the Fall and once in the Spring. You will be billed approximately 42% of the annual rate in Fall and 58% of the annual rate in Spring. The Spring payment includes coverage for the summer. 
    • Family member’s coverage is billed directly to you by the Insurance company and cannot be charged to the Bursar account.

    Leaving Ball State:

    If you are graduating or completing your program at Ball State, check your immigration documents to ensure that your I-20 or your DS-2019 shows the correct end date. You are not required to maintain the Ball State Insurance after your program ends, but you are still eligible to continue until the end of enrollment period. (Example: if you graduate in December, you can, if you wish, continue on the insurance until August 14 of the next year.) If you want to end your insurance after your program at Ball State has ended, you can meet with an International Student Advisor to complete a withdrawal form and shorten your immigration documents.
