Health Insurance Waiver

If you have Health Insurance that meets the same coverage as the Ball State endorsed plan, you can apply for a waiver.

Waiver Process

  1. Fill out the online waiver form here apply for a waiver. Deadlines for the waiver process are listed below.
  2. Submit scanned copies of your insurance card and your policy brochure or coverage details to student@haylor.com .
  3. You will receive an email from haylor.com confirming or denying your waiver.
  4. Submit the final confirmation email along with the Insurance Waiver Confirmation to the Rinker Center reception desk in Student Center, Room 102.

You will need the following information

  • Your Ball State student ID number and your last name.
  • Be able to show that your insurance meets the following coverage
    • At least $250,000 maximum benefit for accident or sickness
    • Maximum of $5,000 out-of-pocket within the $250, 000 benefit.
    • 100% coverage for accident or sickness
    • Cover for pre-existing conditions within 6 months
    • At least $15,000 for repatriation of remains expense
    • At least $50,000 in medical evacuation expense
  • Individual details such as biographical information and student status information.
  • Insurance details such as Policy/Group number, Addresses of the company, Phone of the company and Effective coverage dates for verification purposes

Waiver Deadlines

You can apply for waivers beginning 6 weeks before the start of classes. You must have applied for the waiver and submitted all required documents by the second Friday of classes. If you are beginning your program in October, March or June, you will need to speak directly with an International student advisor within the first week of classes at the Rinker Center to apply for a waiver.

If you waive the insurance plan and drop or lose coverage on your private insurance plan, you must notify the Rinker Center immediately so that you can be enrolled in the HTH plan.



**Note: if your current policy does not cover Repatriation and Medical Evacuation (#4 and #5), you can get a supplemental policy from this website that will cover it.  Click "Evacuation and Repatriation" on the left menu bar. Click on the “Enrollment Form” Link to enroll.
