Life, AD&D, and Long Term Disability

  1. How do I change my beneficiary on life insurance coverage and with my retirement plan?
    You may print the Life Insurance Beneficiary Change Form (PDF), complete and return it to PEB, and link to the TRF and PERF sites for their forms or come to the PEB office to complete these forms. Some forms require the address, social security number and date of birth of the beneficiary. If your pension is with one or more of the APP companies, you need to contact those companies directly.
  2. How do I apply for Long Term Disability benefits?
    Once you have been off work for 4 weeks, you will receive a letter and brochure with information on how to start the application process by calling a toll free number to CIGNA Group Insurance 1-800-352-0611. If you don't receive this brochure, please contact the Disability Specialist in PEB.
  3. If my Long Term Disability claim is approved how do I receive my benefit checks?
    Long Term Disability benefits are paid on a monthly basis and checks are mailed to the PEB office. When the checks arrive in our office we audit for accuracy and mail the checks to you on the same day they are received.
  4. Are Long Term Disability benefits taxable?
    These benefits are subject to federal and state tax. You will have the option to have taxes withheld by completing the federal and state withholding forms. Please contact the Cigna office for the proper forms. FICA/Medicare tax will automatically be withheld from any benefit for the first 6 months of disability.
